Building Bridges Between Hospitals and Community Partners to Promote Birth Equity and Improve Birth Outcomes

This course provides learners with practical strategies to partner with local community organizations to support linkage of pregnant and postpartum patients to community resources that are aimed at improving health equity. Currently, many hospital systems and community organizations work in silos and would benefit on knowledge to effectively integrate these two entities to improve patient care.

Learning Objectives:

  • Recognize the benefits of community partners to improve perinatal care amongst obstetric providers.
  • Describe the importance of improving integration of community partners (such as doulas, home visiting programs, community health workers, patient navigators in hospital spaces.
  • Recognize the linkage of patients to resources such as home visiting services, mobile health clinics, doulas, and patient navigators.
  • Generate opportunities to engage community partners in perinatal research.
